Change is as Easy as Completing the New Account Kit for
Personal Banking:
- Download and print the New Account Kit for Personal Accounts.
- Complete the kit and send your form through fax at (818) 394-2035 or (661) 775-4180, through mail, or by bringing the form into one of our locations listed below.
- Once we receive your application, we will complete your signature cards and have them ready for you when you visit the branch to open your account (valid primary and secondary ID must be presented at the time of account opening).

We suggest that you complete this form and mail it to each depositor (your employer, Social Security, etc.) with whom you have an arrangement for Direct Deposit. You must inform each sender in order for you to receive proper credit.
